**Athletics Registration Directions**

1. Log into your Focus account
2. On the right-hand side of the parent portal home screen, click on the Forms menu
3. Click on the menu item Athletics Registration
4. Follow the Athletics Registration Form Completion Directives to complete your athlete’s registration for fall sports.

**Physical Examination Form Directions**

1.     Download the Physical Examination Form

a.     Physical Form in English

b.    Physical Form in Spanish

2.     Family Members must complete page two before submitting pages two and three to your child’s doctor.

3.     Once forms have been collected from your child’s doctor, submit all completed pages to the portal for approval.  

Important: Please ensure you read through all steps of the directives and turn in all sections to the Focus Portal.

 **New Athletes**

SportsNet Inc. provides secure online registration services for high school athletic programs. Below are instructions for creating a parent/guardian user account and registering one or more athletes at Liberty High School.

Creating an Account

Go to https://sportsnethost.com/libertyhs-parent

1. Click the Create an Account link near the bottom of the screen
2. Enter the required information and submit the form
3. Log in to the system with your email address and password
4. Follow the instructions inside the system

NOTE: The school will not receive your submission until you complete the entire process, ending with your final electronic signature on the last available screen. Be sure to check your email for a confirmation message after you have finished.
